Job Title: Hospital Risk & Quality Management Coordinator
Job Duties: Proactively evaluate areas of organizational risk based on internal assessment and external benchmarking; and implement strategies and policies, which promote patient and staff safety. Promote the organization-wide RISK (Reporting Incidents for Safe Keeping) occurrence reporting process including, trending and reporting of results, identification of problem prone areas and facilitation of prevention initiatives. Direct the investigation of all potentially compensable events, (preventable adverse events). Lead the review of serious occurrence requiring Root Cause Analysis (RCA) of Failure Mode & Effect Analysis. In Collaboration with the Corporate Quality and Performance Improvement Manager, disseminate lessons learned and process improvement plans effectively throughout the organization. Meet regularly with the Manager of Quality, Nursing and Medical Staff Leaders, and the Administrator to provide detailed reports on all serious incidents, claims and risk related issues. Respond to Sentinel Events and other serious occurrences and provide expert advice in the management and reporting of such events to the administrative and clinical leadership and staff. Conduct immediate/appropriate response to any serious occurrence/complaint representing actual or potential patient, visitor and or employee injury. Monitor and take subsequent actions to assure learning, compliance and documentation. Facilitate Mortality and Morbidity Committee Meetings, including initial review of all deaths preparation and dissemination of agenda, minutes, and other pertinent information to Council members. Initiates physician peer review process and occurrence screening activities. Analyze all statistical reports and advisories that identify risk management and patient safety patterns and trends for leadership, Quality Council Committee, Medical Executive Committee, Pharmacy and Therapeutic Committee and the Governing Board. Ensure maximum protection from discovery of all such materials. Coordinate claims investigation and legal defense processes. Promote Hospital-wide Risk Management and Patient Safety Programs throughout the organization to all staff and physicians, in conjunction with the Quality Management. Keep up to date with new and revised state and federal regulations and statutes related to hospital risk, quality and patient care and safety. Review and evaluate related policies as needed. Create for approval, new policies as needed. Actively participate in a variety of committees within the hospital as assigned. Responsible in conjunction with management for developing, coordinating, implementing, and evaluating the continuous quality improvement activities throughout the hospital according to the established Quality and Performance Management program plan. Ensure hospital’s compliance to federal requirements are met and adhered and monitor progress and plan of corrections. Promote a culture of safety through education, monitoring, analysis, and performance improvement activities. Responsible for facilitating Quality Council Committee Meetings, including preparation and dissemination of agenda, minutes, and other pertinent information to Council members. Coordinate with health service leaders with establishment of indicators for monitoring and evaluating the full spectrum of care and services provided to members for quality, appropriateness, continuous improvement, and satisfaction. Provides education in the area of quality and performance management to all departments and assists in ensuring compliance with regulatory and accrediting organizations. Interpret and analyze statistical information on a monthly and or quarterly basis. Present and/or report findings into a concise monthly and or quarterly. Establishes goals, objectives, policies, and procedures for Quality and Performance Management Program in consultation with the Manager of Quality and Performance Management. Establishes and maintains supportive relationships with all department staff, Managers, and chief to ensure effective of QPM program. Provide adequate security systems to protect confidentiality of patient, staff and hospital information. Provide consultative services to assure organizational compliance with the Quality and Performance Management program. Supervise performance improvement studies to ensure validity of data collection processes. Provides consultative services to clinical departments and services of the Corporation in achieving regulatory and accreditation standards. Develop and maintain schedule of quality and performance improvement monitoring activities for hospital-wide QPM program. Work in collaboration with the coordinators of Utilization Review and Infection Control. Plan and coordinate in service education programs in response to education needs of the hospital staff. Promote problem resolution by timely intervention and reassessment of problems. Complete management assignment within allotted timeframe. Maintain, prepare, create and update quality dashboard for monitoring, implementation and distribution. Maintain Quality/Risk Departmental file with current personnel information. Maintain strict confidentiality of quality files including those under investigative review. Work collaboratively and provides feedback directly to the Manager of Quality and Performance Management, Hospital Director, Director of Nursing etc. regarding problem areas, status of quality and performance improvement results of corrective action and reassessment of problems. In collaboration with the Manager for Quality and Performance Improvement, annually evaluate risk management and quality/performance management program for improvement opportunities.
High School Diploma Required? Yes
Qualification Requirements: Bachelor of Science in Nursing or four-year healthcare related field from a recognized and accredited university required. Minimum of five years clinical experience; two years of which in risk and quality management capacity in a hospital setting or minimum of three years experience in a healthcare organization working with quality and risk management within the health or corporate sector; two years of which working as a senior member of a multidisciplinary teams. Previous experience in setting up Hospital Quality Assurance, Quality Improvement, Performance Improvement and/or a Continuous Quality Improvement (CQI) program/Risk Management Program is an advantage. Preferred previous experience in providing education in cross-cultural settings and statistical analysis and reporting. Knowledgeable with Centers for Medicare and Medicaid Services (CMS) inspections, accreditation standards, and/or federal regulations. Certified Professional in Healthcare Risk Management (CPHRM) or Certified Professional in Healthcare Quality (CPHQ) preferred.
