Job Title: Utilization Review/ Case Manager
Job Duties: Reviews continued stay appropriateness on inpatient records concurrently during patients hospital stay. Evaluates patient medical records for accurate and complete documentation of medical necessity, treatment modalities and appropriate level of care. Reviews inpatient medical records for discharge. Identifies planning needs of patients and coordinates all disciplines to assure needs are met prior to discharge. Facilitate the Discharge Planning meetings with the QIO in collaboration with the home health agencies. Durable Medical Equipment is arranged/prepared prior to discharge to ensure continuity of care. Evaluates patient medical records for accurate and complete documentation of medical necessity, treatment modalities and appropriate level of care. Communicate to Utilization review coordinator any discrepancies in documentation of services. Determines medical necessity of hospitalization using evidenced based criteria.Writes statistical and narrative reports to Utilization review Coordinator on outcome of audits of provider services, highlighting areas such as timeliness of provider denials, certified admissions and extensions, and administrative stays. Participates in site certifications to meet regulatory standards and serves as member of monitoring team in periodic program reviews. Refers clients to appropriate local quality care resources. Uses an electronic health record system. Maintain a log of chart reviews and other tracking mechanism. Participates in Hospital Quality Management Program. In the absence of the Utilization Review Coordinator, must perform duties and responsibilities as delegated. Performs other job-related duties as assigned by UR Coordinator and/or Director of CQPM.
High School Diploma Required? Undetermined
Qualification Requirements: Graduation from an accredited Nursing Program, BSN preferred. Minimum 5 years post graduate nursing in an acute facility. NCLEX license required. Current CNMI Registered License. Excellent communication skills needed Computer proficiency preferred. Ability to collect and calculate rates and statistics. Should possess good listening skills and strong time management skills. Ability to work independently with minimum supervision. Must possess good organizational and communication skills.
