| Job Duties: | Provides physical therapy services to home bound patients in accordance with the Plan of care and agency policies and procedures Restore function, improve mobility, relieve pain and prevent or limit permanent physical disabilities of patients suffering from injuries or disease. Treat patients, establish a care of plan or contribute to an existing plan. Provide consultation within their expertise and determine when patients/clients need to be referred to another healthcare professional. Undertake a comprehensive examination, assessment and evaluation of the patient/client or needs of a client group. Formulate a diagnosis, prognosis and plan. Implement a physical therapist interventions/treatments program. Consult with physicians and other healthcare clinicians. Documents all findings, plans, interventions and outcomes in the clinical records. Teach patients and their families how to manage their illness or injuries. Explaining post-treatment home care needs and make recommendations for self-management. |

| Qualification Requirements: | 1.) Must have a degree in Bachelor of Science in PHYSICAL THERAPY 2. Must be a LICENSED PHYSICAL THERAPIST IN ANY US – territory and state jurisdiction 3.) Must be able to accomplish treatment plan and operate therapeutic devices. 4.) Must be able to undertake a comprehensive examination, assessment and evaluation of the patient and formulate a diagnosis, prognosis and plan.
