| Job Duties: | 1.Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist a.Range of motion exercises b.Assistance in ambulation or exercises 2.Performs personal care activities, including but limited to: a. Bathing b. Shampooing c. Skin and nail care d. Oral hygiene e. Shaving f. Dressing 3.Performs household services essential to health care at home, including but not limited to: a. Light meal preparation and feeding b. Laundry c. Light housekeeping 4.Assists in the administration of oral medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and agency policy) 5.Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse 6.Document care provided. Complete forms required in the client’s records. Complete the appropriate records for documentation of care given with pertinent observations. 7.Promotes personal safety and a safe environment for clients. Observe infection control practices. Follow agency guidelines, and report unsafe situations to the Supervisor/Case Manager 8.Demonstrate safe practice in the use of equipment. Do not use equipment until orientation has been provided. Notify supervisor or educational needs. 9.Communicate effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences as requested. 10.Maintain confidentiality in all aspects of the job 11.Participate in required in-service programs to meet compliance requirements. Identify learning needs to agency supervisor. 12.Perform other related duties and responsibilities as deemed necessary. |

| Qualification Requirements: | 1.Successful completion of a formal certification training program and/or written skills test and competency evaluation 2.Licensed to practice as a Nursing Assistant in the Commonwealth of the Northern Marianas islands. 3.At least eighteen (18) years of age 4.Minimum of six (6) months work experience in a supervised setting, preferably in a health care facility 5.Demonstrated ability to read, write, and follow a written Plan of Care 6.Good interpersonal skills 7.Current driver’s License, good driving record, and reliable transportation. |

| Comments: | Home Health Aide visit is paid at the rate of $15 per visit that is not less than 60 minutes with documentation. Minimum Visits could include performing care as prescribed in the Plan of Care as well as any other services required by the patient in completing the visit.
