| Job Duties: | Provides physical therapy services to home bound patients in accordance with the Plan of care and agency policies and procedures • Restore function, improve mobility, relieve pain and prevent or limit permanent physical disabilities of patients suffering from injuries or disease. • Treat patients, establish a care of plan or contribute to an existing plan. • Provide consultation within their expertise and determine when patients/clients need to be referred to another healthcare professional. • Undertake a comprehensive examination, assessment and evaluation of the patient/client or needs of a client group. • Formulate a diagnosis, prognosis and plan. • Implement a physical therapist interventions/treatments program. • Consult with physicians and other healthcare clinicians. • Documents all findings, plans, interventions and outcomes in the clinical records. • Teach patients and their families how to manage their illness or injuries. • Explaining post-treatment home care needs and make recommendations for self-management. |

| Qualification Requirements: | 1.) Must have a degree in Bachelor of Science in PHYSICAL THERAPY 2. Must be a LICENSED PHYSICAL THERAPIST IN ANY US – territory and state jurisdiction 3.) With 5+ years work experienced as PHYSICAL THERAPIST 4.) Must be able to accomplish treatment plan and operate therapeutic devices. 5.) Must be able to undertake a comprehensive examination, assessment and evaluation of the patient and formulate a diagnosis, prognosis and plan.
