Job Title: Hospital Quality Coordinator
Job Duties: Develop, coordinate, implement and evaluate the continuous quality improvement activities throughout the hospital, according to the established Quality and Performance Management Program and in conjunction with management. Ensure hospital’s compliance to federal requirements are met and adhered and monitor progress and plan of corrections. Establish goals, objectives, policies, and procedures for QPM Program in consultation with the DQPM. Establish and maintain supportive relationships with all department staff, directors, and chief to ensure effectiveness of QPM program. Provide education in the area of quality and performance management to all departments and assistance to ensure compliance with regulatory and accrediting organizations. Provide adequate security systems to protect confidentiality of patient, staff, and hospital information. Provide consultative service to assure organizational compliance with the QPM program. Provide consultative services to clinical departments and services of the Corporation in achieving regulatory and accreditation standards. Plan and coordinate in service education programs in response to educational needs of the hospital staff. Interpret and analyze statistical information on a monthly and/or quarterly basis. Present and/or report findings into a concise monthly and/or quarterly report and at requested managerial meetings. Facilitate Quality Council Meetings, including preparation and dissemination of agenda, minutes, and other pertinent information to Council members. Facilitate and supervise performance improvement studies to ensure validity of data collection process. Facilitate incident reporting system. Develop and maintains schedule of quality and performance improvement monitoring activities for hospital-wide QPM program. Develop annual budget for QPM, Infection Control and Utilization Review. Initiate physician peer review process and occurrence screening activities. Assist health service leaders establishment of indicators or monitoring and evaluating the full spectrum of care and services provided to members for quality, appropriateness, continuous improvement, and satisfaction. Promote problem resolution by timely intervention and reassessment of problems. Promote a culture of safety through education, monitoring, analysis, and performance improvement activities. Complete management assignment within allotted time frames. Maintain, prepare, create and update quality dashboards for monitoring, implementation, and distribution. Maintain QPM department files with current personnel information. Maintain strict confidentiality of quality files including those under investigative review. Work in collaboration with the Utilization Review Coordinators and Infection Control Preventionist. Work collaboratively and provides feedback directly to the Director of Quality and Performance Management, Hospital Administrator, Director of Nursing, etc. regarding problem areas, status of quality and performance improvement results of corrective action and reassessment of problems. Serve as a coordinator for mortality review, as well as facilitate the process. Perform all other work related duties, shift, on-call as assigned.

Qualification Requirements: Bachelor’s degree in Nursing or other heath care related field from a recognized and accredited college or university required. Minimum of three years project management experience preferably in Quality Assurance, Quality Improvement and/or Performance Improvement activities. Minimum of at least five (5) years experience with working knowledge of hospital processes, process improvement theory, medical terminologies and accreditation regulations and/or federal regulations (CMS or JCAHO) required. Experience in providing education in cross-cultural settings is an advantage. Possess strong organizational and time management skills. Effective verbal and communication skills. Ability to work in groups and interact with all relevant components of the healthcare system. Self-directed with strong decision making skills. Able to adapt to cultural diversity and sensitivity towards target population. Computer literate e.g. Microsoft Office Suite and database management skills. Must hold RN license or must possess licensure within six (6) months of hire. Certification in Quality preferred.
