| Job Duties: | POSITION SUMMARY: Under supervision of a registered nurse, provides quality care to patients and performs technical skilled care in patient's home in compliance with nursing standards, federal, state, local government regulations and company clinical policies. 1.Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or Registered Nurse designee. 2.Ensures quality and safe delivery of home health services to patients [clients and families/ caregivers. 3.Implements plan of care formulated by physician and Registered Nurse. 4.Provides accurate and timely documentation consistent with plan of care. 5.Assesses and provides patient/ client and family caregiver education and information pertinent to diagnosis and plan of care. 6.Maintains safe and healthy working environment and use safe work methods and procedures in accordance with company clinical standards. 7.Participates in coordination of home health and/ or clinical services with Registered Nurse and clinical management. 8.Assists patient in learning appropriate self-care activities. 9.Uses equipment and supplies effectively and efficiently. 10.Documents and reports activity as required. 11.Performs other reasonable related duties as assigned by management. |

| Qualification Requirements: | 1.Licensed Practical Nurse or Licensed Vocational Nurse in State of Practice. 2.Graduate of an accredited state-approved practical or vocational nursing program. 3.At least one year of nursing experience; community/home health experience preferred. 4.Excellent verbal and written communication skills and strong interpersonal skills. 5.Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. 6.Current driver's license, safe driving record, and reliable transportation. |
